Welcome to the website of On The Move Care, LLC (“On The Move Care,” “we,” “us,” or “our”). These Terms & Conditions govern your access to and use of our website, service request forms, communications, and other online features.

By using this website or submitting a service request, you agree to these Terms & Conditions.

1. Our Services

On The Move Care provides mobile healthcare and support services in the St. Louis area and surrounding communities.

Services may include:

  • Mobile phlebotomy and specimen collection

  • In-home care and personal support

  • Senior and disability transportation

  • Mobile notary services

  • Related support services

Availability may vary by location, staffing, service type, scheduling, documentation requirements, and other operational factors.

2. Service Requests Are Not Confirmed Appointments

Submitting a service request form does not create, reserve, or confirm an appointment.

A member of the On The Move Care team will generally contact you within 48 hours to review your request, discuss availability, and coordinate scheduling.

No service is considered confirmed until On The Move Care has directly communicated confirmation.

3. Mobile Phlebotomy Services

Mobile phlebotomy services may include routine blood draws, specialty collections, specimen handling, specimen pickup or delivery, occupational collections, and related services.

Some collections may require a valid laboratory order, requisition, collection kit, or instructions from a healthcare provider or laboratory.

Clients are responsible for providing any required documentation before or at the time of service.

On The Move Care may decline or reschedule a collection if required information, supplies, orders, or instructions are unavailable or incomplete.

4. No Diagnosis or Medical Treatment

Unless specifically stated otherwise and provided through appropriately authorized personnel, On The Move Care does not provide medical diagnosis, prescribe medication, interpret laboratory results, or establish treatment plans through this website.

Questions regarding medical conditions, laboratory results, treatment, fasting requirements, medications, or medical necessity should be directed to the appropriate healthcare provider.

5. In-Home Care Services

In-home care services may include companionship, meal preparation, medication reminders, light housekeeping, mobility support, personal care assistance, respite support, and other non-emergency supportive services.

The exact scope of service will depend on the client’s needs, service arrangement, staff availability, and applicable requirements.

Services that require a higher level of clinical care may not be available unless specifically arranged through qualified personnel.

6. Transportation Services

Transportation services are intended to provide supportive transportation for seniors, individuals with disabilities, and other eligible clients.

Clients are responsible for providing accurate pickup and destination information, mobility requirements, accessibility needs, and any other information necessary to coordinate transportation.

Transportation availability may be affected by distance, weather, traffic, vehicle availability, accessibility needs, and scheduling.

On The Move Care may delay, reschedule, or decline transportation when conditions make travel unsafe or unreasonable.

7. Notary Services

Notary services are subject to applicable Missouri law and identity-verification requirements.

Clients are responsible for:

  • Providing acceptable identification

  • Ensuring documents are complete and ready for notarization

  • Providing required witnesses when applicable

  • Understanding the contents of documents before signing

On The Move Care does not provide legal advice and does not determine whether a document is legally sufficient or appropriate for a particular purpose.

A notary may decline a notarization when identity cannot be verified, required parties are unavailable, the signer appears unwilling or unable to proceed, or applicable legal requirements are not met.

8. Pricing and Additional Fees

Prices may vary depending on service type, travel distance, urgency, number of clients, complexity, after-hours requests, weekend or holiday scheduling, and other service requirements.

Additional fees may apply for:

  • Travel

  • After-hours service

  • Weekend or holiday service

  • Wait time

  • Additional mileage

  • Additional collections or supplies

  • Specialty service requirements

Any applicable charges will be communicated when reasonably possible before service is performed.

Prices may be changed at any time.

9. Payment

Payment may be required before or at the time of service unless another arrangement has been approved.

Accepted payment methods may vary.

Clients are responsible for all agreed charges associated with requested services, including applicable travel, add-on, or after-hours fees.

10. Cancellations and Rescheduling

Clients should provide reasonable notice when an appointment must be canceled or rescheduled.

On The Move Care may establish cancellation, late-cancellation, no-show, or travel policies based on the service being provided.

If a provider has already traveled to the service location, certain travel or service fees may still apply.

11. Client Responsibilities

By requesting service, you agree to:

  • Provide accurate contact and service information

  • Provide a safe and reasonably accessible service location

  • Disclose relevant mobility or accessibility needs

  • Provide required documentation
